Depois de seu confronto com a Besta em inFamous, Cole McGrath se vê forçado a partir em retirada, já que suas habilidades não são poderosas o suficiente para garantir a vitória. O protagonista segue até New Marais, uma cidade do sul do país que lembra o clima da Nova Luisiana, antes da passagem do furacão Katrina.
Foi nesta cidade que a Ray Sphere foi concebida pela primeira vez, mas Cole terá trabalho para alcançar outra, já que uma milícia criada e organizada por Bertrand não vê com bons olhos aqueles com habilidades sobre-humanas. Além das barreiras humanas, o jogador terá que superar também os mutantes de pele escura que varrem cada esquina de New Marais.
A continuação oferece aos jogadores uma série de melhorias em relação ao episódio anterior, começando pela variedade do cenário, que agora apresenta cores por todos os cantos e uma retratação bem mais fiel da vida urbana, com cabines telefônicas, mesas e muita movimentação pelos terraços e calçadas.
Tudo isso poderá ser destruído, graças ao novo sistema de destruição, que preserva somente as construções mais pesadas, mediante o uso das habilidades novas do personagem. O tornado elétrico, por exemplo, é capaz de derrubar helicópteros, enquanto suga mesas, portas, pessoas e outros objetos inanimados que estão pelo caminho.
O sistema de decisões também retorna, com perguntas ainda mais impactantes que as vistas ao longo da primeira história. Quem participou na definição da narrativa e na própria construção das cenas animadas internamente foi a Naughty Dog, responsável pelo jogo de plataforma e ação, Uncharted.
